FISAA CLUB Membership Agreement
Premium Membership Agreement
WHEREAS, FISAA Corp. offers to provide certain benefits to its members, including discounts and other privileges with participating merchants of goods and services; and
WHEREAS, Member wishes to utilize the benefits that FISAA Corp. will make available to its members; and
NOW, THEREFORE, the parties hereby mutually agree as follows:
1. Member of FISAA receives a medical insurance with the Assurant Health Insurance Company. FISAA has right to change an insurance company if other American Insurance Company will provide better plan for members of FISAA. (Information of the medical coverage and rules and regulations are in the addendum 1).
2. In case of initiation of legal charges (traffic law, criminal law, immigration, family law, real estate, business law) against Member in USA, Europe and other countries (for details see an attachment), Association will pay an attorney’s fee (any court fee, filing fee, etc. are excluded). An attorney will handle the case until the matter of dispute is completely resolved. Member will receive legal help only for legal charges that occur after he/she became a Member. No legal help will be provided for legal charges that were discovered after the membership is expired or canceled. Member shall inform an operator of FISAA about an incident that caused initiation or criminal charges against Member in 24 hours. The description of incident and any documents associated with an incident shall be sent via fax or e-mail. Member shall receive a written confirmation of receiving of the documents from an operator of FISAA. Member shall keep copies of all documents sent to FISAA.
3. Member has right to register his/her computer and get a free service:
(A) Computer performance evaluation; System upgrade and troubleshooting; Upgrades Windows and Windows media player, etc.; Memory upgrade; Antivirus programs stronger than “Norton”; System backup; Technical support over the phone; Cleaning of computer, software and hardware including fans.
(B) Repair and change spare parts (discount is 60%).
4. In case of necessary departure to the countries of South America, Africa and Asia Member has right for necessary vaccinations in the company Passport Health. The discount for service is 20% from regular price.
5. Member of FISAA receives travel insurance with European insurance company (for details see an attachment).
6. Member is entitled to receive discounts for goods and services of companies – sponsors and partners of FISAA (for details see www.fisaa.com or the booklet “FISAA Today”):
- Member is entitled to special program of discounts in Dental office (additional information is on the site www.fisaa.com or in the booklet “FISAA Today”).
- Member is entitled to special program in the office of Acupunctural medicine and the discount is 30% (address is on the site www.fisaa.com).
- Мember is entitled to buy pair of prescription glasses with discount (address is on the site www.fisaa.com or in the booklet “FISAA Today”).
- Member is entitled to discounts in Zion Clinic Pharmacy (address is on the site www.fisaa.com or in the booklet “FISAA Today”).
- Member is entitled to a special discount for service of Plastic Surgeon and medical Rejuvenation (additional information is on the site www.fisaa.com or in the booklet “FISAA Today”).
- If Member is not able to drive a motor vehicle due to health condition he/she is entitled to order a car service and an assistant to go to a hospital once a year. Member shall call operator of FISAA 48 hours prior needed time and vehicle can be used for not more than 4 hours.
- Member is entitled to financial aid of $250 in case of car accident. Member will not be obligated to pay membership fee for next two month from the date of accident (police report shall be presented).
- If Member has suffered from a car accident, and the damages are more than $1,000, Member will not be obligated to pay membership fee for next two months (police report and estimation of damages shall be presented in 24 hours from the time accident occurs). Member shall use car repair shop that is a member of FISAA (additional information is on the site www.fisaa.com or in the booklet “FISAA Today”).
- Member can obtain an auto insurance including an additional discount for FISAA members.
- Member can receive service at Salon and Spa (company – sponsor of FISAA) with guaranteed price of $58.00 per session for facials and $43.00 per session (1 hour) for full body massage. An appointment shall be made through the operator of FISAA.
- To assist Member with career opportunities, FISAA will allow Member to post his/her resume on its website.
- To assist Member with business opportunities, FISAA will allow Member to promote his/her products, goods and/or services on its website.
- To assist Member with advertising opportunities, FISAA will allow Member to advertise the sale of his/her real estate, motor vehicle, and/or other personal property on its website.
- An appointment for visiting a doctor or a hospital shall be made through an operator of FISAA only.
- If Member suffered injuries due to car accident or at work and does not have the possibility to work for more than two months thus he/she may get a financial aid in the form of interest free loan in the amount of $800 per month to the complete recovery, but not more than 15 months. Such loan issued for a period of 5 years and can be paid prior the end of the term of the loan by parts or in full. The loan shall be initiated by an additional agreement. The recipient of the loan shall be Member of FISAA for an entire period of loan. In case of the early cancelation of membership, Member is obligated to pay off the entire loan.
- FISAA has service of maintenance and repair of personal computers (additional information is on the site www.fisaa.com or in the booklet “FISAA Today”).
- Member authorizes FISAA or its assigns to charge his/her bank or credit card in the amount of $179.00 every month.
- Member agrees that he/she is not entitled to a refund for previously charged monthly membership fees.
- If, in case of emergency, Member has to get to the hospital and the hospital’s authority refuses to accept Member’s insurance FISAA will take care of situation.
- Member is obligated to open an account in Sun Trust Bank.
7.Member understands and agrees that any and all companies listed in FISAA’s directory offering goods and services at discount rates and/or other privileges to members of FISAA are acting and performing entirely independent from FISAA, and FISAA in no way approves, endorses, guarantees and/or warrants those companies or products, and, therefore, Member hereby waives any and all claims he/she may have against FISAA.
8. Member is aware that FISAA is in no way responsible for the content and/or reliability of any advertisement or other information other members or individuals may from time to time post and disseminate on FISAA’s website, and, therefore, Member hereby waives any and all claims he/she may have against FISAA.
9. Member agrees and will indemnify FISAA and hold it harmless from any and all claims, actions, damages, liability and expenses in connection with or arising out of Member’s or his/her agent’s activity with FISAA.
10. This Agreement shall continue to be in full force and effect until terminated by either party upon delivery of prior written notice.
11. This Agreement shall not be changed orally, but only by an Agreement in writing, signed by the party against whom enforcement of any waiver, change, modification, extension or discharge is sought.
12. Should any covenant, condition, term or provision of this Agreement be deemed to be illegal, or if the application thereof to any person or in any circumstances shall, to any extent, be invalid or unenforceable, the remainder of this Agreement, or the application of such covenant, condition, term or provision to persons or in circumstances other than those to which it is held invalid or unenforceable, shall not be affected thereby; and each covenant, condition, term and provision of this Agreement shall be valid and enforceable to the fullest extent permitted by law.
13. Except as herein provided to the contrary, this Agreement shall be binding upon and inure to the benefit of the parties hereto and their respective executors, administrators, personal representatives, successor trustees, successors and assigns.
14. The failure of either party to insist, in any one or more instances, upon performance of any of the terms or conditions of this Agreement shall not be construed as a waiver or relinquishment of any right granted hereunder or of the future performance of any such term, covenant, or condition, but the obligations of each party with respect thereto shall continue in full force and effect.
15. This Agreement is intended by the parties to be their complete agreement and all other terms, agreements, conditions and representations are merged. No representations, inducement, promises or agreements, oral or otherwise, between the parties not embodied herein, shall be of any force or effect.
16. This Agreement has been entered into in the State of Florida, and the validity, interpretation and legal effect of this Agreement shall be governed by the laws of the State of Florida.
17. A fully executed facsimile copy of this Agreement shall have the same force and effect as an original.
18. This Agreement may be executed in one or more counterparts, each of which shall be deemed an original, but all of which together shall constitute one and the same agreement.
19. This Agreement shall continue to be in full force and effect until terminated by either party upon delivery of prior written notice 30 days before the requested termination date.
20. In case of failure to execute any conditions of the agreement, membership shall be canceled.
IN WITNESS WHEREOF, the parties hereto have duly executed this Agreement on the date set forth below.
